What medical conditions qualify for a MMJ card in Arizona?
Currently Arizona recognizes 13 medical diagnoses as qualifying conditions for a MMJ card.
What are the steps for getting my MMJ card in Arizona? You must have a medical record within the last 12 months that shows an evaluation by a doctor for a qualifying condition. Many medical marijuana certification clinics will have a specialist on staff to perform this evaluation at an additional fee. If your primary doctor is unable or unwilling to certify you as being qualified for a medical marijuana card, you must find one who will. This is typically done through certification clinics sponsored by dispensaries or doctors who will charge a fee for their service. Once you recieve your certification, you will need to fill out the state application for your MMJ card and pay the state fee ($150, or $75 if you are on food stamps). Most clinics offer to submit this application for a small processing fee, or you are able to do it directly from the Arizona Department of Health Services website.
